  5. 5. A period of transformation and renewal <ul><li>In the past three years The Scarborough Hospital has worked hard to improve performance on all fronts. This focused effort has resulted in some remarkable achievements, including: </li></ul><ul><li>Industry leading infection control practices </li></ul><ul><li>Best hand hygiene rates in the province </li></ul><ul><li>Improvements in the Hospital Standardized Mortality Rate (HSMR) that now place TSH amongst the top performers in the GTA </li></ul><ul><li>Improvements in patient satisfaction (still an area of focus) </li></ul><ul><li>A new MRI at the Birchmount campus and planning approval on new operating rooms at the General campus </li></ul>

What is a Clinical Action Plan? <ul><li>A clinical action plan is designed to identify long-term areas of growth and focus for the hospital based on projected population needs. </li></ul><ul><li>It does not change core hospital services such as emergency services. </li></ul><ul><li>It is a five to ten-year plan. </li></ul>

How was this plan created? <ul><li>The process of creating the plan involved staff, physicians, patients and the community </li></ul><ul><li>It looked at the “ideal patient experience” as a basis for planning </li></ul><ul><li>Decisions were based upon: </li></ul><ul><ul><li>strategic fit </li></ul></ul><ul><ul><li>alignment with external directives </li></ul></ul><ul><ul><li>ideal patient experience </li></ul></ul><ul><ul><li>clinical impact </li></ul></ul><ul><ul><li>community needs </li></ul></ul>

What was approved? <ul><li>2 Strategic Proposals </li></ul><ul><ul><li>Cancer (Create a Comprehensive Cancer Program at TSH) </li></ul></ul><ul><ul><li>Chronic Disease Management (Develop a framework to deliver comprehensive care for chronic diseases at TSH) </li></ul></ul><ul><li>4 Tactical proposals (Patient Experience/Safety) </li></ul><ul><ul><li>CCRT for Birchmount </li></ul></ul><ul><ul><li>Expand Hospitalist Model </li></ul></ul><ul><ul><li>Improved Pediatric Emergency Care </li></ul></ul><ul><ul><li>Regional Anesthesia & Acute Pain Program </li></ul></ul>
