  2. 2. Healthcare at the Precipice <ul><li>Lifestyle choices of modern society has increased the incidence of obesity and associated diabetes, hypertension, hyperlipidemia and vascular disease. Renal and heart failure are becoming progressively more rampant. </li></ul><ul><li>The cost of new diagnostic and therapeutic technology is rising so steeply that the economic growth of most countries is unable to keep pace with a growing and sicker population </li></ul>

Healthcare at the Precipice <ul><li>Yesterday’s healthcare model is too expensive and is begging restructuring via innovation </li></ul><ul><li>Congruently, physicians are getting frustrated and a shortage of specialists is looming in the USA as they face more reimbursement cuts </li></ul><ul><li>We must find a way to increase the reach of specialists so that they can provide remote consultation at the point of care </li></ul>

Healthcare at the Precipice <ul><li>Priorities within the limits of finite resources & evidence-based guidelines must be determined & implemented by the Specialists </li></ul><ul><li>Unbiased Evidence-based Decision making and advise is not a form of “rationing”. </li></ul><ul><li>It is based on solid data and may avert an inevitable “famine” of healthcare if we in Healthcare are not proactive. </li></ul>
  5. 5. Telemedicine using the MacGuyver* Approach * = MacGuyver was a TV Series that ran bewteen 1985 and 1992 Telemedicine is the component that allows Specialists to remotely interview patients and to discuss care with the Healthcare provider at the remote facility

Telemedicine Components <ul><li>Skype Account = Free </li></ul><ul><li>A fast broadband internet connection means smooth video calls Cost = ~$70 - $100/month </li></ul><ul><li>Total Initial Hardware Cost, including labor = Less than $1200 + Less than $100/month for High Speed Internet Connection per point of care such as the Emergency room </li></ul>
  8. 8. Further Leveraging the Internet <ul><li>Use of Electronic Medical Records (EMR) to generate and exchange patient clinical information. The system must be: </li></ul><ul><li>Pure browser based with use of Cloud Technology </li></ul><ul><li>Intuitive and easy to use. Relatively inexpensive. ONC-ATCB certified for Meaningful use </li></ul><ul><li>Powerful integrated logic so as to dispense with restrictive templates </li></ul><ul><li>Use of clinical diagnoses that are coupled to ICD-9 /ICD-10 billing codes. Billing codes alone do not provide sufficient & specific information about a disease state </li></ul><ul><li>Ability to scan documents and images for subsequent review </li></ul><ul><li>Secure messaging for exchange of information </li></ul>
  9. 9. Further Leveraging the Internet <ul><li>Provide Online Education/Training with use of GoToMeeting or alternative web conferencing software at around $50/month. Facilitate Live Interaction with multiple Healthcare providers in different locations </li></ul><ul><li>Use websites to provide patients and healthcare providers with educational information. Also, allow them to review previously recorded lectures </li></ul><ul><li>Allow access to online textbooks and online electronic review articles to promote the practice of evidence-based medicine ( UpToDate .com ) </li></ul>
