Information provided on the form should be corrected or filled in if missing. These forms need to be returned with proof of residence attached as soon as possible. Proof of residence may be a water bill, electric bill, lease or mortgage statement. Phone and cable bills are not acceptable proof of residence. Your student’s name and grade should be hand- written in the upper left corner of the proof of residence with both first and last names provided. Included is the picture form for fall school pictures. An application for free and reduce lunch is also being sent home for those who would like to apply. All items are to be returned to your student’s first period teacher. Thank you for your assistance in this matter. We look forward to a positive and productive year. Our goal is to help your student excel in school while providing a safe, secure and welcoming educational environment. Sincerely, Susan Carpenter Principal
