Go to www.cms.gov/EHRIncentiveprograms
Select Registration and Attestation

Register
Click on link to Register

Click through the About window
Click to continue

Log in
Physician to enter the NPPES
User ID and password

Go to Registration tab
Select Registration tab

Scroll down and click “register”
Click to Register

Click through the Registration steps
Click to Start Registration

First step
Select program type
Pick EP type from pick-list
Click “’yes”
Enter Practice Fusion’s Certification Number: 30000004H0RCEA0
(All ‘0’ are zeros)

Second step
Select who is to receive the $

Fill in additional info
Fill in info on $ recipient

Third step
Verify info, add email

Finished - submit
Complete – begin submission

Verify and submit
Verify and submit

Accept, agree and submit
agree

Finished!
