  2. 2. Improving our safety culture one floor at a time!   Why do we need a Huddle? Safety is everyone's responsibility so how can we improve communication? How can we better communicate risks and issues to all team members? How did we start a Huddle?  Pilot the Safety Huddles on one unit.  Develop and refine the data collection tool.  Pick a time and location that works for everyone. 5 minute maximum time limit!  Keep it safety simple! (K.I.S.S.)
  3. 3. The Safety Huddle! (5 Minutes to Improved Safety) What is discussed?  Medication issues  Equipment Malfunction  Falls/Slips/ Trips  Wound and Skin Integrity  Infection Control  Behaviors  Safety/ Security   The Safety Huddle Tool is hung in a central location so that everyone can read it or add to it. Tool is reviewed daily to ensure follow up on all issues and ensure plan development .
  4. 4. Excellent results!   Improved communication identified on trial unit. Implemented on all 4 units successfully with same enthusiastic response!    Staff feel more empowered and informed. Communication on safety issues has dramatically improved. Safety Huddles are now imbedded in our daily practice.
