Consider starting the advance care planning process if not already in place Published April 2010 Resident has changing condition requires medical review Assess Needs Not End of Life but greater need (months) ! Regular Assessment Review monthly ! Discuss with Multi Disciplinary team place resident on Supportive Care register ! Apply for Continuing Care Funding ! Order equipment via Social Care Resident has deteriorating condition Assess Needs End of Life very urgent (Weeks/Days) ! Discuss with GP & District Nurse ! Regular Assessment ! Review daily ! Request any equipment as urgent End of Life user ! Consider request to Marie Curie for support using Continuing Care Funding ! Commence Liverpool Care Pathway or equivalent
