2006: Midwest Cluster needs Collaborative effort
Pilot project developed
Six teams from six states participated
Five of the six reported positive results
2007 – 2009: Has undergone numerous revisions to adapt to program needs

2008- -2009 Project RevUP Historical Perspective
MPCA interest continued
RevUP revised to meet logistical and funding requirements
Nine applications received
Six centers selected
Timeframe: September 2008 – September 2009

The Project Included:
Initial one day face-to-face session with team members from all centers
Intense 2-day site visit at each participating center
Direct coaching by MPCA staff
Joint conference and individual calls for training, coaching and sharing
Interim “report out” at Clinicians meeting
Final one day learning/sharing session with team members from all centers
