      • 2006: Midwest Cluster needs Collaborative effort

    • 2008- -2009 Project RevUP Historical Perspective
      • MPCA interest continued
      • RevUP revised to meet logistical and funding requirements
      • Nine applications received
      • Six centers selected
      • Timeframe: September 2008 – September 2009

    • The Project Included:
    • Initial one day face-to-face session with team members from all centers
    • Intense 2-day site visit at each participating center
    • Direct coaching by MPCA staff
    • Joint conference and individual calls for training, coaching and sharing
    • Interim “report out” at Clinicians meeting
    • Final one day learning/sharing session with team members from all centers
