Vorteile
• Anerkannte Therapie
• Gut wirksam bei
Strahlentherapie
Nachteile
• Operation notwendig
• Unwirksam bei
Chemotherapie
15. Kryopräservation von Embryonen oder
Oozyten
Transfer nach abgeschlossener Therapie
Vorteile
• Gute Erfahrungswerte
• in vielen Zentren
durchführbar
Nachteile
• Teuer
• Nicht durchführbar bei sehr
jungen Frauen ohne Partner
• Mögliche Aggravierung der
Grunderkrankung bei
östrogenabhängigen
Erkrankungen wie Brust-Ca, SLE
• Mässige Erfolgsrate
16. Ovarian Tissue Banking
- Grosse Anzahl an Primordialfollikeln im Gewebe
vorhanden – höhere Erfolgschancen?
- Keine Stimulation aber LSK notwendig
- Kurzfristig durchführbar (Op Freigabe)
- Natürliche Empfängnis denkbar
- Als autologe Hormonersatztherapie denkbar
19. Ovarian Tissue Banking
- Einschlusskriterien -
primäres Kriterium: Erhaltung der Fertilität
- Altersgrenze: < = ca. 37 a
- FSH < 10
- LSK möglich
- Keine anamnestische Bestrahlung
- Frühes Stadium der Chemotherapie
20. Ovarian Tissue Banking
- Ganzes Ovar oder Teilovarektomie ?-
Ovarektomie:
Lokale oder Ganzkörperbestrahlung
Tangentiale Resektion:
Chemotherapie ohne Bestrahlung
21. Kryopräservation von ovariellen
Scheiben, Biopsien
Vorteile: - Einfacherer Friervorgang
- Einfachere Ex- und Implantation (LSK)
- Orthotope Position ermöglicht
= natürliche Empfängnis
- Implantation wiederholbar
Nachteile: - Geringere Menge an Primordialfollikel
- Verlust in der Revaskularisierung

37. Conclusio
• Ovarialprotektion ist sinnvoll
• Wahlweise GnRH Analoga oder OTB
• OTB vor allem bei hormonsensitiven Tumoren,
Bestrahlung und jungen Patientinnen
• GnRH-a Langzeitergebnisse noch unklar
• OTB ist vor allem die Technik der Reimplantation
und das Risiko der Tumorübertragung nicht
geklärt
Exploratory analysis conclusions Importantly, treatment allocation (Femara ® or tamoxifen) was key to the clinical outcome and affected the chances of a patient receiving breast-conserving surgery. While baseline tumor size did not affect clinical response rates, it was a significant factor in predicting whether a patient could undergo breast-conserving surgery. Indeed, patients with T2 tumors were 4.5 times more likely to undergo breast-conserving surgery than those with tumors >T2.
