For items that do not apply indicate N/A inthe space provided. Where patient numbers are requested, estimate what you expect will occur. If anyrequested information is not available, an explanation should be given and it should be so indicated in theappropriate place on the form.Once the forms are complete, number the pages sequentially in the bottom center.
