  • What is the SCR? • The SCR is an electronic summary of key health information. It will hold limited essential information (medication, adverse reactions and allergies) derived from the patient‟s GP record • Additional information (e.g. care plans) may be included at the request of (or with the explicit consent of) the patient • Patients will be asked for „Permission to View‟ their SCR before it is accessed
  • • 30.5 million people in England have an SCR (more than 50% of the population) • Records created by 4250 GP practices • 44.5 million patients contacted across 210 CCGs • Opt out rate 1.38% National Rollout of the SCR
  • SCR National Rollout October 2013 KEY No Activity PIP Commenced Record Creation Commenced Over 60% Records Created Over 80% Records Created
  • Areas Viewing the SCR currently More than 8000 SCRs are being viewed each week in the following care settings: • 131 GP Out of Hours care settings • Pharmacists in 99 hospitals • 18 hospital Emergency Departments • Acute admissions wards in 29 hospitals • 32 Community & Intermediate Care teams • 43 Walk in Centres/Minor Injuries Units • 3 GP practices for temporary residents
  • Preparing for wider use of the SCR: It is anticipated that the SCR will be increasingly used in other care settings by healthcare staff working independently &/or as members of a wider team including: • Ambulance Services • Urgent care outside of hospitals • GPs for temporary residents Other care settings are also emerging – eg: • Mental Health • 111 Services • Hospices • Offender Health • ………..
  • SCR Benefits – High Level • Patient Safety reducing the risk of prescribing errors and adverse reactions to prescribed medication • Increased efficiency and effectiveness reducing time, effort and resource required to share information across different NHS organisations • Increased Quality of Patient Care enabling the most appropriate care to be delivered in the most appropriate setting
  • Analysis of data over a year from one OOH provider (>70000 encounters) • Patients are less likely to require a face to face appointment in OOH when the SCR has been viewed in a phone consultation • Patients are less likely to be asked to contact their GP surgery for a follow up appointment when the SCR is viewed
  • Use of the SCR by Hospital Pharmacists • Sheffield: The SCR reduced average time for medicines reconciliation from 2 hrs 9 mins to 19 mins • Leeds: Time taken to complete medicines reconciliation has reduced by 55% - now only taking 19 minutes; the team is now meeting the 24 hr target 87% of the time (previously 57%) • “Pharmacists love the SCR because it makes their life a lot easier.   • Enabling Viewing of the SCR isn‟t hard • SmartCards for authorised staff (and readers on computers) • Secure N3 connection • Some training - mainly the Information Governance aspects • Patients need to be asked for „Permission to View‟ their SCR (if unable to give permission because not mentally competent, confused or unconscious – staff should use „Emergency Access‟ in the patient‟s best interest) • The trust needs to identify a „Privacy Officer‟ who can audit emergency access and any self-claimed access to ensure that there is no malicious or inappropriate viewing • And that‟s it ….
  • The Future of the SCR “We have commissioned the HSCIC to add immunisations, significant past problems & procedures, end of life care information & other patient preferences to the SCR…… ...the SCR will provide NHS Trusts with a key building block towards an IDCR and we strongly recommend its uptake and adoption.” NHS England July 2013
