Partnership Overview <ul><li>Mission: Milwaukee Health Care Partnership is a Public/Private Partnership dedicated to improving health care for Underserved Populations in Milwaukee Co </li></ul><ul><li>Founded: January, 2007 </li></ul><ul><li>Membership: 5 Milwaukee based health systems, 4 FQHCs, Medical College, State, County and City health agencies </li></ul><ul><li>Affiliates: WHA, WPHCA, Medical Society, Free and Community Clinic Network </li></ul><ul><li>Goals: Improve Health, Health Outcomes and Reduce the Total Cost of Care Delivery </li></ul><ul><li>Priorities: </li></ul><ul><ul><li>Secure Coverage for All - Badger Care (Medicaid) and Enroll those who are categorically eligible </li></ul></ul><ul><ul><li>Increase Access </li></ul></ul><ul><ul><ul><li>Primary Care : Build the capacity and capability of the Milwaukee FQHCs </li></ul></ul></ul><ul><ul><ul><li>Specialty Access : Improve access for Medicaid and uninsured </li></ul></ul></ul><ul><ul><ul><li>Medication Access : Leverage free and discounted medication assistance programs </li></ul></ul></ul><ul><ul><ul><li>Behavioral Health Access : Improve the Milwaukee County BHD; commission study </li></ul></ul></ul><ul><ul><li>Improve Care Coordination – Reduce primary care ED utilization and connect patient to medical homes </li></ul></ul>

Outcomes <ul><li>Coverage: </li></ul><ul><ul><li>Over 24,000 new BC+ enrollees in 2008 </li></ul></ul><ul><ul><ul><li>(Children and custodial parents at &lt; 200% FPL) </li></ul></ul></ul><ul><ul><li>12,300 new BC+ Core Plan enrollees as of 1/1/2009 </li></ul></ul><ul><ul><ul><li>(Childless Adults &lt; 200% FPL) </li></ul></ul></ul><ul><li>Access: </li></ul><ul><ul><li>Over 20 new FQHC net providers </li></ul></ul><ul><ul><li>13% increase in Medicaid and uninsured patients served </li></ul></ul><ul><ul><li>$1.6 million in new health system funding to create FQHC medical homes for uninsured </li></ul></ul><ul><ul><li>Implementation of IT supported PAP program in 10 community clinics </li></ul></ul><ul><ul><li>Elimination of ED diversions for Behavioral Health patients and a significant reduction in ED wait times for IP admission </li></ul></ul><ul><li>Care Coordination: </li></ul><ul><ul><li>Over 600 ED primary care referrals to FQHCs </li></ul></ul><ul><ul><li>Funding and implementation of ED IT Linking providing health information exchange across 10 Milwaukee EDs </li></ul></ul>
