    1. Partnership Overview
      • Mission: Milwaukee Health Care Partnership is a Public/Private Partnership dedicated to improving health care for Underserved Populations in Milwaukee Co
      • Founded: January, 2007
      • Membership: 5 Milwaukee based health systems, 4 FQHCs, Medical College, State, County and City health agencies
      • Affiliates: WHA, WPHCA, Medical Society, Free and Community Clinic Network
      • Goals: Improve Health, Health Outcomes and Reduce the Total Cost of Care Delivery
      • Priorities:
        • Secure Coverage for All - Badger Care (Medicaid) and Enroll those who are categorically eligible
        • Increase Access
          • Primary Care : Build the capacity and capability of the Milwaukee FQHCs
          • Specialty Access : Improve access for Medicaid and uninsured
          • Medication Access : Leverage free and discounted medication assistance programs
          • Behavioral Health Access : Improve the Milwaukee County BHD; commission study
        • Improve Care Coordination – Reduce primary care ED utilization and connect patient to medical homes
    2. Outcomes
      • Coverage:
        • Over 24,000 new BC+ enrollees in 2008
          • (Children and custodial parents at < 200% FPL)
        • 12,300 new BC+ Core Plan enrollees as of 1/1/2009
          • (Childless Adults < 200% FPL)
      • Access:
        • Over 20 new FQHC net providers
        • 13% increase in Medicaid and uninsured patients served
        • $1.6 million in new health system funding to create FQHC medical homes for uninsured
        • Implementation of IT supported PAP program in 10 community clinics
        • Elimination of ED diversions for Behavioral Health patients and a significant reduction in ED wait times for IP admission
      • Care Coordination:
        • Over 600 ED primary care referrals to FQHCs
        • Funding and implementation of ED IT Linking providing health information exchange across 10 Milwaukee EDs
