You then have the ability to access one or multipleprograms demographics and/or actionable items:

Let’s walk through an intake path:1. Who are you? I am a 24 year old married female with 2 children.2. Have you been in WFS before? Yes, I was in an ICC program in 2008.3. Why are you here today? I am pregnant Prenatal Intake & Case ManagementWell Family System directs theparticipation path based on Intake Type.

Then a Plan of Care is initiated based upon the input data: Info at Intake is analyzed and triggers needs in several categories. Which automatically builds a basic PHYSICAL & FSP MENTAL HEALTH CATEGORY ITEM ACTION Physical and Mental Health Dental Issues Refer to Dental Services Early Education & No Infant Care Enroll with Pediatrician Development Safe & Nurturing Environment No Permanent Home Housing Assistance Social-Emotional Economic Stability Father Not Working Refer to WIC Job Training Development Spiritual Foundation A Plan of Care is initiated and The Family Support Plan provides a Safe & Nurturing starting point for determining a path of Environment care and can be shared across programs and can be part of the Economic Stability family history over time.

You can then access the individual from multiple agencies to view all services: MANAGE SHARED SERVICESWFS allows collaboration of services andinformation between multiple programs.The most common example is sharedservices or assessments. More than oneagency may provide parentingeducation, etc. or administer an ASQ. WFS provides common services to be viewed by multiple agencies to help reduce duplication of services. Further rules can be established that determine which agency can act on the required service.

…and also view family connections.MANAGE FAMILY CONNECTIONSWhere permissions are granted to single or multiple programs the WFS Family Connection posts anotice when a family member, other than the one being served in a specific program, is opened as acase in another program. A typical instance may be where there is a prenatal woman in oneprogram and the father of the baby joins a male participation program. Over time a typical examplemay be an older woman, once a teen mom, in an interconceptional care program where herdaughter entered Healthy Start as a prenatal intake, etc.
