With immense pleasure I extend a heartiest welcome to all the guests and delegates participating in the 54th Annual Conference of the Indian Society of Hematology and Blood Transfusion being held in Mumbai from November 6-10, 2013. Hematology as a subject had been slow to take off in our country but recent years have seen an increase in the training as well as treatment facilities. We need to grow at a rapid pace all over the country in order to cater to the needs of the society. There is a need to popularize the subject among the young undergraduates and postgraduates. The society is making efforts in this direction by organizing annual quiz competition throughout the country. We as a society of professionals need to look at problems peculiar to this country and develop management paradigms that suit our needs and would help people with resource constraints. The society has in the recent years joined hands with American Society of Hematology and European Hematology association and organized joint academic programs. I hope that it will go beyond this and open doors for close collaboration in research and training. Other Asian societies are also keen to forge close bonds with the society. This year Prof MB Agarwal has organized a tremendous academic feast and has some of the best known names in hematology as guest faculty. I am sure that the interactions in this meeting will not only help us update knowledge but also give us opportunity to develop long term academic and social relations. It is indeed my pleasure and privilege as President Elect (2013) of ISHBT to invite my honorable fellow colleagues to participate in HAEMATOCON 2013, the 54th Annual Conference of ISHBT, being held in Mumbai, the financial capital of India, from 7th to 10th Nov (with Workshop on 6th Nov), I would like to congratulate Dr M B Agarwal, the Organizing Secretary and his team for planning a very interesting and relevant programs covering all aspects in the growing field of Haematology and upgrading this to a mega event this year. I welcome all the delegates and send my best wishes to the participants and organizers and hope it will be another hall mark in the annals of ISHBT as it marches ahead in its quest for sustained development and I am sure this Scientific Extravaganza under the able leadership of Dr Agarwal will provide an excellent opportunity to the participants in exchange of ideas, knowledge and experience which will help in taking this discipline of Medicine ahead in our country. Agarwal. The scientific programme has been planned meticulously. It offers a rich academic content by eminent national and international faculty to deliberate upon the advances in Haematology, which I am confident, will be an academic feast for all the delegates.   11. 11. It is a matter of great pleasure and pride to present special issue of the journal on Haematocon 2013, Annual conference of Indian Society of Hematology & Blood Transfusion. The conference issue this year has a unique significance in the sense that it has set a record both in number and quality of abstracts. We have research papers on diverse aspects including clinical, applied and basic hematology. Furthermore a well crafted educational programme consists of lectures by renowned National and International experts. Dr M B Agarwal and his team have done a commendable job and they deserve applaud.   13. 13. First of all I would like to thank Dr. M.B. Agarwal and the organizing committee for inviting me to be the “Guest of Honor” at this prestigious 54thAnnual Conference of ISHBT. I am glad to note that there are more than 1500 delegates registered; nearly 400 abstracts submitted; and deliberations going on in 10 halls simultaneously. It just shows the organizational skills of Dr. M.B.Agarwal and that the field of Hematology has really come of age in India. Anemia is still the leading cause of “disease” in India and a simple diagnosis and treatment can make a big change in the life of the patient. However, we have still not been able to prevent it and in many cases treat it early. The same holds true with heredity disease like Thalasemia. Even, though we have facilities to do prenatal diagnosis, we have still not been able to prevent the births of children with Thalasemia Major in this country. I think we should be working more actively in the field of Preventive hematology in a developing country like India. Since I am an Oncologist, I will delve more on aspects of hemato-oncology. In a way Hemato-oncology has been the leader in various fields. It has paved the way in formulating the principles of therapy in medical oncology. It has helped us understand the biology of various malignancies and showed us how molecular biology can impact both-the understanding of biology as well as developing effective treatment protocols. The field of targeted therapies and personalized medicine started with hematological malignancies likeAcute Promyelocytic Leukemia and Chronic Myeloid Leukemia. Rituximab paved the way of use of various other monoclonal antibodies including Trastuzumab in breast cancer and Cetuximab in H & N and GI cancers. The use of Minimal Residual Disease has helped us best risk–stratify our patients with acute leukemia, especially Acute Lymphoblastic Leukemia, and thus decrease treatment for those who can be cured with minimal therapy and at the same time increase treatment early in patients with high risk disease. This has not only helped us to better utilize our limited resources, but also to improve overall outcomes in children withALL. Even at TMH we have given a lot of emphasis and infrastructure for taking care of hematological malignancies. At TMH we registered nearly 4000 new patients with hematological malignancies last year, which must be the highest number for any Institution in the subcontinent. We treat the maximum number of leukemia patients in the country. A lot of support, including blood bank and financial, is provided for these patients. We have a state-of-the art 6 bedded Stem Cell Transplant unit atACTREC, Kharghar, which we hope to expand in the near future.Anew 200 bedded Adult Hemato-Oncology facility will be developed at ACTREC and a 100 bedded Pediatric Hemato-Oncology facility will be developed across the road in the Haffkine Premises at Parel in the next few years. We already have a state-of the art Cytogenetic and Molecular Diagnostic and Research facility at TMH. The hem- path division is doing MRD studies for both ALL and AML patients. The management is fully supporting financially challenged patients for life-saving SCT procedures and 62 such patients have received SCT in the last 3 years. The pediatric Hem-Onc DMG is another unit where 80% of patients are treated in general category with great financial and holistic support from TMC. Most of our treatment protocols are indigenously developed as per local needs and they have improved the overall survival between 70% & 90% for most childhood hematological malignancies. However, being from a developing country, I would like to end by talking about the important lessons we have learnt from treating hematological malignancies. First, in regard to the treatment of childhood ALL: The outcome has increased from <10% in 1960's to >90%. presently using the same molecules that were discovered in the 60's thru 80's. This has taught us that we really do not need to run after newer molecules everyday for improving the outcomes. We can improvise with the time tested molecules itself. The other part of this story is also that this has been achieved by treating patients on National & International Collaborative trials. We have to work in collaboration and uniformly treat every child on protocols so as to get maximum useful information in regard to biology and outcome of the disease. The same holds true for Burkitt's Lymphoma.And secondly, that more is not always better. At a time when treatingAcute Promyelocytic Leukemia was considered very difficult and more patients withAPL in the West were treated with higher dose therapies including transplants, a developing country like China introduced 2 simple therapies likeATRAand laterArsenic, which not only substantially improved the outcomes ofAPLpatients but also at a much reduced cost. We have to think Globally, but act Locally! So I wish you have a great meeting over the next 2 days and I hope there will be many fruitful discussions and hopefully forming of at least some National Collaborative groups as well as discussions on locally relevant Research which will help improve the outcome of patients with various hematological conditions in this country.
