Italian government is in charge of the overall coordination of the Health System. All health care treatments (excluding dental) are covered by the National Health system.   6. 6. What we tend to have Full capacity to share information and documents: amongst hospitals and between specialists and GPs. Capacity to rationalize administrative processes and make the information technologies “mission critical” in healthcare sector. Identify an interoperability standard used in the hospitals and by all healthcare professionals and make compatible, possibly uniform, the informatics structure of Hospital Information Systems. CRS–SISS overviewThe core of the project is an ‘HealthcareExtranet’, which links professionals, socialservices, organizations and citizens, tracking allthe events in the process of patient care andproviding value added services.The project is based on smartcard technologies, grantingaccess to the Network to bothcitizens and operators by theirpersonal smart cards. Objectives&Functionalities Information sharing and exchange amongst healthcare Citizens operators and professionals to improve quality of healthcare provision (consultation referrals, prescription process, etc.). Improvement of services provided to Citizens through an easy and secure access to the Healthcare System.Administration Information availability at analytical level to monitor healthcare quality and expenditure during the Public process of care for a better planning at Regional level (management, budget control, expenditure). Two joint implementations CRS (“Regional Services Card”): the personal smart card allow every citizen to access all public administration services and enabling to access to private services. SISS (“Health Care Information System”): the regional network connect all healthcare and social providers in Lombardy and provide e- Health services for ensuring continuous care.   11. 11. SISS major services Prescription management Electronic prescription: drugs and services coding, control, digital signature, storage, display. Healthcare document management Outpatient examination report, Letter of Discharge, Emergency Form: digital signature, notification, display, import in professional’s EPR. Issuing and maintenance of report are in charge of the health care company providing the service. Outpatient booking service Through Unique Regional Call Center or through a software used by GPs and Chemistries. Network Providers they are the intermediaries between users and the regional trust that are in charge for the project implementation; they are selected and certified by the Region Lombardy; they provide the access to the SISS Network and to Internet; they install the “front end” software and the “SISS programs” on the operator’s Work Stations; they supply the first level “Help Desk” and the “on site” technical support for the SISS; they are chosen on the market and paid by Hospitals, GPs and Pharmacists. Lessons learnt The cost of developing the entire informatics system is a small part of the total investments: we spent for that about 70 mil €; The main economical effort is to upgrade the Hospital Information System: we have already spent for that 250 million €; The security infrastructure cost is smaller then the “Privacy” respect: we spend 200.000 € for security and 20 million € per year for “Privacy”; The training cost is less that we could wait; At present, the health professionals accept Regional Healthcare Information System and they are using it more and more.
