    • Meals on Wheels – 90,000 meals delivered every year in the Tri-City Area
    • Friendly Visitor – Over 150 volunteers provide companionship through supportive visits, phone calls and outings

    • V.I.P. Rides – Volunteers provide assistance to seniors traveling with Paratransit on in the volunteers car
    • Falls Prevention – In-home exercise program, coached by LVN students

  • 4. Falls by seniors have a major impact on quality of life…
    • Leading cause of injury death of individuals over the age of 60
    • #1 reason for entry into long-term care facilities
    • Over 1.3 million older Californians fall each year… many sustain hip fractures, head injury or death. Falls are the leading cause of loss of independence among seniors
  • 5. Every year… one-third of older adults fall
      • 300,000 will fracture a hip . . .
      • and within 1 year …
    • … 100,000 of these will have died as a result
    • Of those who survive many will have some form of reduced mobility and will have problems functioning independently

    • There’s no charge for this service… the only thing we ask is that the client do their exercises, as best they can, five days a week. In that way, they will see the greatest improvement in balance and gait.
  • 12. What do you have to do to participate? It’s simple

    • 60 years or older
    • Live in the Tri-City Areas of Fremont, Newark or Union City
    • No heart, hip or knee surgery within the past six month
    • Those who can’t get out of the house to exercise on their own
    • It’s just that simple!
