  • Senior Practitioner – Operational leads.Qualified Practitioner – Supports clients on their transition into adulthood through the SDS process. The number of qualified practitioners has increased to 12 from 2011 when there were only 4.Support Broker – Facilitates support planning where there might be particularly high support needs or there are communication difficulties.Carers Worker – Specific responsibility for supporting and advocating for carers.OT – Will be assessing equipment needs and supporting with behaviour and sensory needs, and independent living skills.
  • Transition Protocol will also explain the roles and responsibilities of the Transition Team and other professionals during this time.
  • Financial Assessment (Paying for adult community care services):If we provide you with community care services you might need to contribute towards the cost of your care package. The amount you will be asked to pay will depend on your income, savings and expenditure. It will also depend on the type of service you are receiving. Community Care includes home based care, Self-directed Support, major adaptations, and day care.You or your representative will be asked to complete a financial assessment form, so that we can work out how much, if any, you will have to pay. We will help you to complete the form and answer any questions you might have.The rules on charging for community care services can be complicated, so we have produced a leaflet which is called: Paying for Care - home and community services, to help you understand how we work out if you will pay.Our policy on charging for community services is based on guidance issued by the Department of Health. This is called: Fairer Charging Policy 2010.
  • Supported Self Assessment:This Supported Self Assessment will help us to understand and identify what needs the young person has so that we can work out how much support they need. Most of the questions help us do this, and will calculate the personal budget. Some questions will act as triggers to other more detailed assessments, as appropriate, for example OT assessments or Continuing Health Care Assessments.Supported Self Assessment is in two parts – The Young Person & The CarerFirst part is for the young person to tell us about their needs around personal care, daily living activities such as cooking, cleaning, shopping and managing money, leisure activities, keeping safe, making decisions etc. Second part looks at the impact on the Carer, and takes into account their lifestyle, health and well-being.Supported Self Assessment is about choice and control for young people and their families. It is person centred and needs led.
  • Resource Allocation & Indicative Budget:After completing Supported Self Assessment the young person will be informed of their Personal Indicative Budget Allocation.Once the young person knows how much their personal budget is, they can begin developing their support plan to meet their needs with people they choose for example; their family, a broker or transition worker.
  • Support Plan:An individual can use their Personal Budget in a way that best meets their needs, for example:To access community activitiesTo provide support at home or in the communityTo provide short breaksSupported LivingThe council wants the young person to be in control where possible. This depends on what they feel able to do, and what they want to do, their level of understanding and their wishes. Support is offered so that they can access advice and support to the degree they wish when developing their plan.There is a process where a young person lacks capacity, which can be discussed with the Transition worker.Managing PBContracted serviceDirect payment(& Indirect payment)Individual service fundSupported Managed Account
