  • Care & Support –Service titleKey Achievements of Innovation inHousing and HealthSara BeamandHead of Older Peoples Service
  • Our Inspiration  We provide specially designed housing for older people both in retirement housing with support and Extra Care housing with domiciliary care services.  Our aim is to support older people to maximise their independence, choice and dignity, we do this through: - Design - Activity / Customer involvement - Care and support - Well Being Services - Staffing
  • Our Impact Experience and research shows that good Quality Extra Care Housing:  Identifies a quarter of residents moving in reduce their care package needs within 3 months of moving in  Residents have a lower likelihood to be admitted to hospital for an overnight stay  Extra care housing translates to fewer falls  Living within an extra care setting with a positive impact on health through changing care packages could potentially equate to a saving of £5,432 per resident per year
  • Our Inspiration – going forward  Develop new Extra Care housing schemes  Continue to provide services which enable older people to maximise their independence  Develop our learning and expertise with innovative methods of working with older people with Dementia Our call for action: As we see an increasing population of people living longer how do we build on our success of supporting people with chronic illness and dementia to reduce hospital stays and maintain a level of independence?
