สำหรับธุร│กฎเกณฑการพิจารณารับประกัน Employees) 4 กิจที่มีพนักงาน 11-30 คน (For Business with 11-30 คุณสมบัติของกลุมที่มีสิทธิสมัครเอาประกัน ธุรกิจขนาดเล็กที่ประกอบดวยจำนวนพนักงานประจำ 5 - 10 คน ผลประโยชนการประกันภัยจะใหความคุมครองแกพนักงานที่มีอายุ 15 - 59 ป ซึ่งปฏิบัติงานเต็มเวลาในวันที่เริ่มมีผลบังคับความคุมครอง ตามกรมธรรมประกันภัยกลุม ระดับขั้นอาชีพ ผลประโยชนการประกันภัยจะใหความคุมครองแกธุรกิจที่มีความเสี่ยงไมเกินระดับอาชีพขั้น 2 เบี้ยประกันภัย อัตราเบี้ยประกันภัยจะขึ้นอยูกับอายุของผูเอาประกันภัยแตละคน ณ วันครบรอบปกรมธรรมของแตละป เบี้ยประกันภัยจะตองชำระเปนรายป หลักเกณฑในการเขารวมเปนสมาชิก พนักงานทั้งหมดจะตองเขารวมทำประกันภัย พนักงานที่มีสิทธิเอาประกันภัยทั้งหมดจะตองกรอกใบสมัครแบบแถลงสุขภาพตามแบบฟอรมของบริษัทฯ วันที่เริ่มมีผลบังคับตามกรมธรรมประกันภัยจะเปนวันถัดไปหลังจากที่บริษัทฯไดรับเอกสารประกอบการสมัครครบถวน และอนุมัติการรับประกันภัยแลว กรณีที่มีพนักงานเขารวมทำประกันภัยระหวางปกรมธรรม วันที่เริ่มมีผลบังคับตามกรมธรรมประกันภัยจะเปนวันที่ 1 ของเดือนถัดไป หลังจากที่บริษัทฯ ไดรับเอกสารประกอบการสมัครครบถวนและอนุมัติการรับประกันภัยแลว การเลือกแผนประกันภัย พนักงานที่มีสิทธิเอาประกันภัยและอยูในตำแหนงงานเดียวกัน จะตองสมัครทำประกันภัยแบบแผนเดียวกัน 1 กรมธรรมสามารถประกอบดวยแผนประกันไมเกินกวา 3 แผน (กำหนดแผนประกันไดสูงสุด 3 แผน ตอ 1 กรมธรรม) ความแตกตางระหวางแผนประกันจะตองไมเกินกวา 3 ระดับแผน ตัวอยาง กรณีที่เลือกแผนประกันที่ 1 เปนแผนประกันต่ำสุด ดังนั้นแผนประกันสูงกวาที่สามารถเลือกไดถัดไปคือ แผนประกันที่ 4 ผลประโยชนคารักษาพยาบาลแบบผูปวยนอก เปนผลประโยชนที่นายจางสามารถเลือกทำเพิ่มเติมได แตหากมีความประสงคที่จะสมัครทำประกันภัย แบบผูปวยนอกเพิ่มเติม พนักงานจะตองเขารวมทำประกันทุกคน พนักงานสามารถเลือกผลประโยชนความคุมครองขามแบบแผนประกันไดสำหรับคารักษาพยาบาลแบบผูปวยนอก กฎเกณฑการสมัครทำประกันภัยใหกับคูสมรสและบุตรของพนักงาน ผลประโยชนคารักษาพยาบาลจะใหความคุมครองแกคูสมรสของพนักงานที่มีอายุต่ำกวา 60 ป และบุตรที่มีอายุไมต่ำกวา 2 อาทิตย และไมเกินกวา 18 ป และยังมิไดสมรส ทั้งนี้ จะขยายความคุมครองใหแกบุตรที่มีอายุระหวาง 18 ป จนถึง 23 ป โดยเปนผูซึ่งกำลังศึกษาเต็มเวลา และยังมิไดสมรส คูสมรสและบุตรที่มีสิทธิเอาประกันภัย จะตองสมัครเอาประกันภัยภายใตแบบแผนเดียวกับพนักงาน (เฉพาะประกันสุขภาพเทานั้น) นายจางเปนผูรับผิดชอบชำระเบี้ยประกันภัยของพนักงาน คูสมรส และบุตรทั้งหมด กรณีที่นายจางตองการสมัครทำประกันภัยสุขภาพใหแกคูสมรสและบุตรของพนักงาน พนักงานที่มีคูสมรสและบุตรจะตองสมัครทำประกันภัยทุกทาน คูสมรสและบุตร ที่มีสิทธิเอาประกันทั้งหมดจะตองกรอกใบสมัครแบบแถลงสุขภาพตามแบบฟอรมของบริษัท เอกสารประกอบการสมัคร ใบคำขอเอาประกันชีวิตกลุมสวัสดิการพนักงาน (สำหรับนายจาง) สำเนาหนังสือรับรองการจดทะเบียนการคาเปนนิติบุคคล หนังสือแจงความประสงคขอใชสิทธิตามโครงการบัตรสิทธิพิเศษประกันภัยกลุม (ถาสมัคร) ใบสรุปรายละเอียดขอมูลของพนักงาน คูสมรส และบุตรทั้งหมด ใบสมัครของพนักงานแบบแถลงสุขภาพ โดยพนักงานเปนผูกรอกรายละเอียดทั้งหมด ใบสมัครของคูสมรสและบุตรแบบแถลงสุขภาพ (ถาสมัคร) สำเนาบัตรประชาชนของพนักงาน คูสมรส และบุตรแตละคน เช็คชำระเบี้ยประกันภัย สั่งจาย "บริษัท อเมริกันอินเตอรแนชชั่นแนลแอสชัวรันส จำกัด" หมายเหตุ เอกสารนี้จัดทำขึ้นเพื่อเสริมสรางความเขาใจเทานั้น จึงมิใชสวนหนึ่งของสัญญาประกันภัย หรือเปนเอกสารผูกพันบริษัท ขอกำหนดและเงื่อนไขของความคุมครองจะระบุไวในกรมธรรมประกันภัยที่ออกให

Underwriting Guidelines │ 5Eligibility A Small Business group is comprised of 5 - 10 full-time employees. All benefits are available to eligible employees age 15 to 59 years who are actively at work on the effective date of the group insurance.Occupational Class All benefits are available to the business with risk exposure not higher than the occupational class 2. (White & Light-blue Collars only)Premium The premium rate is based on the attained age of each insured at each policy anniversary. Mode of payment is annual.Participation Requirements All employees in a company must participate in the group insurance program (On compulsory basis). All eligible employees are required to complete the Health Declaration Form. The effective date is the following day after all required documents are obtained and the insurability is approved. In case that a new employee requests to participate in the group insurance program during the policy year, the effective date is the 1st day of the following month after all required documents are obtained and the insurability is approved.Classification of Plan All eligible employees who are in the same or equivalent position will be insured under the same plan. One policy can consist of not more than 3 different plans. The difference between the insurance plans should not exceed 3 plan levels. For example In case that Plan 1 is chosen, the higher plan must not exceed Plan 4 Clinical Benefit is optional for the employer, in case that the employer decides to take the coverage of Clinical Benefit all eligible employees must participate in the coverage. Crossing of plans for different benefits is allowed for out-patient only.Eligibility of Dependent Only medical benefits are available to spouse below age 60 and child(ren) at least 2 weeks old and not over 18 years of age and unmarried.Child(ren) can be extended from 18 to 23 years old if still a full-time student and unmarried. The eligible dependents must enroll under the same plan as the insured employee (For medical coverage only). The premiums of all eligible employees and their dependents must be paid by the employer. In case that the employer requires to extend medical insurance coverage to the employees dependent, all eligible dependents of all married employees must be insured.
