O documento discute deficiência de hormônio do crescimento (GH) em crianças, jovens e adolescentes. TAMBÉM APRESENTAR UMA DIMINUIÇÃO DA RESPOSTA DO
GH AOS SECRETAGOGOS.
Elas devem ser interpretadas levando-se em conta a nutrição, status
psicossociais e idade óssea. O IGFBP-3 é GH-dependente e caso suas
concentrações também se encontrem baixas, consistem em evidências de
deficiência de GH-hormônio de crescimento em comparação com a
determinação isolada do IGF-1. A Growth Hormone Resarch Society
estabeleceu critérios que tentam regular os diagnósticos de deficiência de
GH-hormônio de crescimento na fase da criança, infância, juventude ou
adolescência apesar da variabilidade dos métodos. Esses critérios usam os
achados clínicos de várias condições associadas com deficiência de GH-
hormônio de crescimento, a severidade da baixa estatura longitudinal ou
linear, o grau e a duração da redução da velocidade de crescimento a fim
de identificar indivíduos que possam possuir deficiência de GH-hormônio
2. de crescimento. Apesar das semelhanças estruturais marcantes entre o
hormônio do crescimento de diferentes espécies, apenas os hormônios do
crescimento humano e de primatas têm efeitos significativos nos seres
humanos. Várias formas moleculares de GH circulam. Grande parte do
hormônio de crescimento-GH na circulação está ligada a uma proteína
(proteína de ligação ao hormônio de crescimento, de GHBP) que é
derivada do receptor do hormônio de crescimento. O GH é secretado no
sangue pelas células somatotróficas da glândula pituitária anterior, em
quantidades maiores do que qualquer outro hormônio da pituitária. Os
níveis de secreção são maiores durante a puberdade. O fator de
transcrição PIT-1 estimula tanto o desenvolvimento destas células e a sua
produção de GH. A falha de desenvolvimento destas células, bem como a
destruição da glândula pituitária anterior, resulta em deficiência de GH. A
Regulação de Peptídeos liberada por núcleos neurossecretores do
hipotálamo para o sangue venoso portal em torno da hipófise são os
principais controladores da secreção de GH pelos somatotrófos. No
entanto, embora o balanço destes peptídeos estimulantes e inibidores
determinam a liberação de GH, este equilíbrio é afetado por muitos
estimuladores fisiológicos e inibidores da secreção de GH.
Os estimuladores da secreção de GH incluem: * hormônio liberador de
hormônio de crescimento (GHRH) a partir do núcleo arqueado * grelina *

proteína dietética * estradiol * arginina. Os inibidores da secreção de GH

glicocorticóides. Além do controle por processos endógenos, uma série de
compostos estranhos (xenobióticos) é agora conhecida por influenciar a
secreção de GH e sua função, com destaque para o fato de que o eixo GH-
IGF é um alvo emergente para determinado desreguladores endócrinos.
As orientações e considerações diagnósticas da declaração da Growth
Hormone Research Society no caso de baixa estatura longitudinal ou linear
pode ser necessário um teste terapêutico com GH-hormônio de
crescimento por DNA-recombinante de 3 a 6 meses para a determinação
da resposta do crescimento; caso o crescimento aumente mais de 2
cm/ano, provavelmente a criança será beneficiada com o tratamento com
GH- hormônio de crescimento, não importando o que os testes
originalmente demonstrem.
