    1. CE and ISO Cleared FDA Pending Study Case Report: Radio-Frequency Assisted Liposuction ™ D. Hurwitz, MD - Pennsylvania, USA Before 6 Weeks After 12 Weeks After Patient: 25 Female with BMI of 39.1 - Class II Obesity Sedative: Vicodin ES, Toradol 10mg, Keflex 500 mg Tumescent: 2,000cc tumescent/local Procedure: Treatment commenced in the deep layer at a depth of 25-40mm (depth setting 5-6). I set my cut-off temperature to 36 degrees centigrade. Unlike the typical fanning motion used in SAL, my technique was modified to ensure that the cannula did not return to the same point, which can result in an over-treated zone. In addition, the movement of the RFAL handpiece moves at a slower pace than SAL, with a complete back and forth cycle spanning approximately 2 seconds. Ideal endpoint is the lack of mechanical resistance in the adipose tissue. Generally, it takes about 3-5 minutes for each zone and the skin temperature will reach 30-34 degrees at this stage. After reaching the deep fat endpoint, the superficial layer was addressed (depth of treatment at 10-15mm; setting 2-3). It took another 3-5min to reach a desired uniform skin temperature. I could move onto the next treatment zone, but I maintain the temperature for a few more minutes for additional tissue contraction. It is critical to observe the skin’s apperance, erythema of the vaso-constricted area indicates a strong skin reaction to the heat and treatment in this area would end. To prevent excessive heat a cool wet gauze can be applied to the pink surface. I have concluded that this heating effect is more typical in high volume patients where a large amount of heat is accumulated in deep layers. Treatment was finished with a traditional 4 mm cannula until a smooth and symmetrical result was obtained. Aspiration: 1800cc (600cc lower right abdomen, 400cc right mid and upper abdomen, 500cc lower left abdomen, 300cc left mid and upper abdomen) Total Energy: 90.8 km/J Weight: 250.0 lbs (before) --> 240.0 lbs (6 weeks) --> 239.0 lbs (12 weeks after) Circumferential: 110.5 cm (before) --> 109.0 cm (6 weeks) --> 108.0 cm (12 weeks after) Post-Op Care: Foam rubber was placed followed by elastic garments.
