En los próximos 30 minutos quiero compartir con vosotros algunas ideas de cómo la tecnología está cambiando en el término más amplio y su potencialidad en la atención a personas en dependencia, presentando casos, tecnologías y experiencias prácticas de Microsoft, haciendo una previsión de evolución a futuro.

Over the past thirteen years Microsoft has steadily increased its investments and commitment to health.
In 2005, the Health Solutions group was created when our corporate VP, Peter Neupert, came on board at the direct request of Steve Ballmer to explore and create solutions for healthcare more deeply. Peter’s been a long time Microsoft employee. He led the OS/2 team and then he orchestrated the MSNBC deal before leaving Microsoft to go on and establish Drugstore.com. After a few years as the CEO of Drugstore.com Peter was on the United States President’s IT Advisory Council when he decided to come back to Microsoft to help us focus very specifically on healthcare and healthcare IT solutions.
Over the course of the following 2 years, HSG purchased key technologies to create a foundation of solutions to address pressing challenges that other IT organizations were not meeting. The result is the solution set we have today – a Family of enterprise solutions, Amalga, and HealthVault, a platform for consumer information.
Today, we have some of healthcare’s leading providers using Microsoft Health Solution Group’s innovative solutions.
Microsoft is the only IT provider simultaneously addressing health information needs from both a consumer and enterprise viewpoint, to advance a vision of unifying health information and making it more readily available, ensuring the best quality of life and affordable care for everyone.

Los modelos familiares en los que se ha basado una gran parte del apoyo y soporte a la tercera edad y a la discapacidad se están desestructurando.
Tenemos unos nuevos tipos de familias y de apoyo social. No podemos contar con el mismo esquema que hace 10 años.
Para conseguir una atención centrada en la persona es necesaria una coordinación entre los servicios sociales y los sanitarios, necesidad de la que cada vez son más conscientes las administraciones públicas (41). Uno de los modelos es el propuesto por el King’s Fund en el Reino Unido (Figura 10) que ha partido de la pirámide de Kaiser, integrando la visión social y la sanitaria “como partes fundamentales de la atención que ha de recibir la persona proponiéndose tres niveles de intervención, formulados a partir de la estratificación de la formación, con objetivos diferenciados” (39).

How often we have to wait for the next appointment with the doctor for a month or so and when we are finally there it takes 15 minutes to go through a checkup? Now imagine a nurse whom you can address anytime from home? She may look a bit strange like Avatar, but she is always for you. By observing patients ability to move, speak and react, she can track the recovery process after a patient was discharged e.g., operation on hip fragmentation, and check if the exercise program is working well, or it may lead to pain increase. The San Francisco development center of France Telecom-Orange has developed Sense.ly platform with Microsoft’s Kinect and Nuance Communications speech recognition engine to engage with patients remotely. Sense.ly collects data from wireless medical devices like tonometer wirelessly connected to Kinect sensors and discuss with a patient who stands in front of a computer or Internet-enabled TV his/her physical conditions.

Groundbreaking PHR application for Europe’s largest mental health service: Fostering collaboration and improving care
Challenge: South London & Maudsley NHS Foundation Trust (SLaM), a premier mental health services provider in the United Kingdom, wanted to develop an interactive patient portal to support its commitment to a new, connected health model. SLaM piloted an “eMPOWERMENT Program" to help patients take a more proactive role in managing their mental health and wellbeing.
Solution: Get Real Health developed MyHealthLocker, a cloud-based application built on the Get Real Health InstantPHR platform, which
leverages the security and privacy standards of Microsoft HealthVault. MyHealthLocker helps SLaM's outpatients to track their mental health and follow the self-management care plan provided by their physician. Outpatients may also complete ongoing assessments
via the portal, and these self-reported measures are then reported back to the care team. The health journal featured in MyHealthLocker is unique and completely customizable to patients, enabling them to track symptoms, sleep patterns, behavior, and emotions—in
any format they choose. Patients can create more detailed journal entries using the innovative calendar, sliders and color-coding features. Mike Denis, the Director of Information Strategy for SLaM, says: “This is not just about giving people access to their health
records. This approach encourages a two-way flow of information between patients and their clinicians.”

Providing coordinated, high-quality care is both facilitated and in some ways complicated by a number of technological trends that are changing the ways many healthcare organizations operate, including:
An Explosion of Data: When I said earlier that most organizations don’t have a clear picture of their patients, it’s not for a lack of information. Today, healthcare organizations are awash with data. What’s worse, it comes wrapped in layers of complex regulations and stringent federal and state regulations (Ex: HIPAA, HITECH Act (US), EU Model Clauses (Europe)). Collecting data is not the problem; it's the ability to process, store and interpret significant amounts of information that is one of today's most important technological drivers in health.
Cloud Computing: Cloud computing is the delivery of IT infrastructure assets over the Internet on a utility basis. Increasing numbers of health organizations are investing in on-premise, cloud, or hybrid solutions to help address their current computing needs, while providing a sustainable path to the future.
Bring your own device: Doctors generally prefer to use the technology with which they are most comfortable. The Bring Your Device trend–in which employees use unsupported, personally owned technology in the workplace—makes this tendency particularly significant for healthcare IT. Healthcare providers are now faced with balancing security in a non-homogenous information technology environment while supporting the different communication needs of workers across an organization. Tablets and new mobility scenarios are now possible in healthcare.
In response to recent healthcare reform initiatives, organizations are leveraging one or more of these trends for data coding, the transmission of data, improved security, quality measurement, safety reporting, electronic health record implementation, meaningful use, accountability, and transparency. Se desarrollan lentes de contacto que detectan los niveles de glucosa en sangre
Un grupo de expertos de la Universidad de Washington ha iniciado el desarrollo de un proyecto con Microsoft sobre lentes de contacto capaces de detectar los niveles de glucosa en sangre de pacientes con diabetes tipo I, diseñadas para proporcionar avisos al profesional médico, y al propio paciente a través de la vista.
"El proyecto desafía nuestra imaginación. Ahora podemos imaginar un mundo en el que lo virtual y lo real se fusionen", según ha indicado Desney Tan, investigador senior de investigación de Microsoft. "Imaginen que su lente de contacto proporcione contenidos digitales adecuados allá donde vaya, imagínese detección médica continua para controlar su salud".
Los investigadores han explicado que el proyecto se encuentra en las fases iniciales. No obstante, el equipo ha diseñado y fabricado ya prototipos válidos de este dispositivo ocular, que cuenta una pantalla LED integrada. Además, se ha contado un enlace de comunicación inalámbrica de datos, así como una unidad de energía, también integradas en este tipo de lentes. Los expertos han indicado que una de las mayores dificultades del proyecto ha sido "hacer responder a la fluctuación de las lentes, centrándose de manera adecuada frente a la luz".
El estudio ya ha conseguido probar las lentes en fluidos lacrimógenos, encontrando resultados positivos respecto a la obtención de los niveles de glucosa en sangre, medidos a través de sensores especiales integrados en la lente. Así, han añadido que "podría servir de una gran ayuda para los pacientes con diabetes de tipo I, lo que les permite controlar sus niveles de glucosa en sangre sin necesidad de pinchar sus dedos varias veces al día".
El objetivo principal del proyecto es el de "registrar la información sobre los niveles de glucosa en sangre para llevar a cabo una revisión continua por parte de los pacientes y sus médicos". Los expertos han indicado que además las alertas de niveles anormales de glucosa podrían llegar a ser mostrados en la propia "vista" del paciente. En este sentido, las alertas requerirían al propio usuario que "se inyectase insulina o comiera algún alimento con contenido en glucosa", mediante avisos visuales.
El equipo está trabajando en el desarrollo de sensores, radios y antenas que puedan ser integrados en el dispositivo, que funcionen de manera efectiva en el ambiente acuoso del ojo, aportando nuevos métodos de recopilación de información del paciente y control de la actividad.
Los expertos han resaltado la importancia de este estudio, que permite al usuario obtener una mejora en su calidad de vida. Han recordado que el avance de la tecnología permite trabajar en proyectos interesantes como este, y que"tras el éxito en la fabricación de ordenadores más rápidos, más pequeños y más baratos, los técnicos y especialistas se centran ahora en la fabricación de otro tipo de informática más accesible, y facilmente integrado en la vida, que permite al usuario hacer cosas más interesantes".
Babak Parviz, profesor asociado en el departamento de ingeniería eléctrica de la Universidad de Washington, ha agradecido al equipo de Microsoft por su colaboración en el proyecto, añadiendo que "mucha gente considera que las lentes de contacto funcionales se trata de ciencia ficción", y recordando que .estos nuevos métodos son de gran interés para los investigadores del campo de la realidad aumentada y la salud continua, sirviendo de inspiración para desarrollar mejores tecnologías en el futuro.
