  • 2.  You were previously asked to put together adraft report to use for the recording ofincidents and accidents. This type oftemplate needs to be filled out accuratelyand according to legal requirements,regulatory requirements and general OHSrequirements.   • 3.  It is very important to ensure thesetemplates are filled in accurately so thatinformation from these forms can betransposed on to reports to be sent tomanagement, senior management, otherstakeholders, insurance companies and anyregulatory bodies.
  • 4.  Organisations need to have appropriaterecord keeping systems in place tostreamline the OHS procedures. Teammembers have an obligation to reportincidents, accidents, potential hazards andnear misses to management.
  • 5.  For this reason, there needs to be adequatecontrol measures in place to ensureinformation is being reported properly andaccording to the required regulations andexpected standards.
  • 6.  Companies should have in place a way todocument, record, verify and improve onOHS information within the workplace. Managers and stakeholders need thereassurance of understanding that theinformation being supplied to them isaccurate and clear, concise and relevant.
  • 7. Staff members being given incorrect information whichcan be detrimental to the organisationError rates going up, cause waste of time for staffStaff members not feeling they can trust other teammembersWaste of time taken to find accurate informationPossible consequences that can be detrimental to otherstaff members and compromise the safety of staffmembers
  • 8.  Written in a clear and succinct manner Kept in a centrally located file so the staffmembers that need access can access Updated often Version controlled so staff members knowwhich is the latest version they are accessing
  • 9. Accident reports – any reports that indicate accidentswhere an employee was hurt or equipment was damagedIncident reports – no matter how trivial or minor anincident seems at the time, all incidents should bereported. This includes illnesses and near missesInjury register – all injuries, no matter how small or trivial,need to be recordedFirst aid register – this register should keep track of allfirst aid administered
  • 10.  Managers need to ensure that theysupport the record keeping process bymonitoring and evaluating the systemsregularly, ensuring the systems arestructured and well organised, that thepolicies and procedures support anyrequired resources and that information isclearly assigned and locatable.
  • 11. It is extremely important that anyinformation provided is accurate,reliable, valid and able to beunderstood by the people that needto analyse the information forfurther use.
  • 12. There are a number of compensationbased statistics that set theparameters form the collection ofstatistics and information relating tothe prevention of injury and illnessin the workplace.
  • 13.  When there are inconsistencies orinadequacies in the reporting systems, orthe risk control plans or risk assessments,management is responsible for the clearcommunication of this to seniormanagement and other relevantstakeholders to make sure further actionis taken from an organisationalperspective.
  • 14. The continuous improvement cycleand process should be followedregularly to make sure all processes,procedures and regulations areimproved and implemented withinthe organisational structure.
