Major Evidences • Impact Project --- pay attention now • Professional Development Plan (PDP) --- start this now – contact Amanda Werts (wertsal) with questions • Praxis II – can’t officially graduate without it – can walk across the stage, but not receive diploma or licensure without it – will get an “I” • Praxis II Code for ASU Office of Field Experiences - 5010

International Student Teaching • RCOE Liaison – Dr. Bill Peacock (peacockws@appstate.edu) • 10 weeks in North Carolina, last 5 weeks abroad • Complete application – 3 reference letters – $150 deposit- turn application in completed – in a manila folder • Informational Meeting – Wed., 9/4/13 at 12 pm in in 430 RCOE

  • 13. Health Clearance • Physical Exam • TB Test – can be conducted at ASU Health Center • Due with other clearances – October 2, 2013 • Make a copy of your health forms for your professional folder

Academic Clearance • Clearance to your program area – signed by program area advisor • Medical Clearance – TB reading appointment • Clearance to College of Education – placed in binder in Lobby of Dean’s Office – College of Education • Sign clearance form in the Dean’s Office • Give form to Laura Anne or slide under HER office door – 404F • Make copies of all forms to keep in your professional folder.
  • 15. Criminal Background Check • Complete Criminal Background Check Form • Give form to Laura Anne or Jean Edwards • Complete on-line portion of CBC when prompted • Complete county criminal background checks if required • Deadline – October 2, 2013 • ASU and Watauga are due immediately
  • 16. Forms • Notecard • Academic Clearance • Drug-testing waiver • Placement Data Sheet • Health Forms or sign-up for appointment • Criminal Background Checks – ASU, Others

Due Dates • All Completed Clearances due to Laura Anne Middlesteadt by Wednesday, October 2, 2013 • You will receive an official placement email from Dr. Robin Groce --- do not plan on a placement or living arrangement until after you receive this electronic letter • Please contact your cooperating teacher once you receive your electronic confirmation letter.
