Comprehensive Wellness Program


Lifestyles Assessments can determine the success of your wellness
program. Our Lifestyles Assessment does more than simply identify
participants at risk for common diseases such as diabetes and heart
disease. Our Lifestyles Assessment can help you:

- reduce healthcare costs by decreasing the number of medical claims for
preventable injuries and illnesses.
  - increases productivity by combating presenteeism and absenteeism.
  - provides a baseline to track health improvements and demonstrate
cost savings.
  - identifies current health risks within your organization, so you can offer
tailored and effective interventions.
  - foster a healthy job environment.

Our simple Lifestyles Assessment, available in paper form or online, is based
on current research in the areas of health and wellness. Once the Lifestyles
Assessment is taken and reviewed by our health coaches, they will receive a
phone call to go over the results.

Health and Lifestyles Coaching



        The Health and Lifestyles Coaching Program includes,
                         but not limited to:

                           Weight Management
                          Exercise Management
                           Tobacco Cessation
                           Stress Management
                                Nutrition
                                Diabetes
                              Heart Health

Each participant - no matter their identified lifestyle situation - will get
           an opportunity to interact with a Health Coach.

Health and Lifestyles Coaching




At Aynes & Brown, LLC, we believe it is essential to the Health and
Lifestyles Coaching program that we outreach to every participant.

Unlike most health coaching programs, that target 20% - 30% of the
 individuals enrolled, we feel targeting 100% of individuals is key
      to the success - that even the healthiest individuals will
       benefit and foster a healthier workplace environment.

Health and Lifestyles Coaching



 Participants will have a number of options regarding how they
     participate in the program. Some individuals will work
one-on-one with a health coach and others will receive periodic
   communication either through telephonic communication,
electronic communication or by the traditional printed material.

    The type of communication method and intervention
  used by our health coaches is decided by the health coach
                  and participant together.

Health and Lifestyles Coaching




  Our health coaches are nurses, or are certified, in a health related
  field such as health care, physical education, sports management,
           nutrition, community health or health promotion.

They participate in ongoing training and quality assurance activities to
 ensure that the highest level of quality coaching is provided to each
                        and every participant.

Our Health and Lifestyle coaches can be used with our Comprehensive
            Wellness Program or your current program.

   Employer reporting is comprehensive and provides valuable
  information. These reports include statistics on the following:

Short Term Disability Management


 Short duration disabilities can be costly to employers, both in lost
  productivity and actual expenses, especially if there is a salary
             continuance plan for short-term absence.

 The Aynes & Brown Short Term Disability Management Program
 offers solutions to assist the employer to maximize the health of
  employees and increase productivity. Our effective approach
   combines integrated disability management with prevention.

We use the most up-to-date Reed MDGuidelines, which are reviewed
                   and updated continuously.

Short Term Disability Management


  We focus on reducing the duration of disability-related absences
         and decreasing costs through early intervention
           and ongoing management of disability cases.

 Our disability nurses work with the employee and specialty team of
           medical professionals. The Short Term Disability
  Management Program monitors the medical necessity for absence,
   guides the employee toward appropriate physician consultation,
educates employees about medical treatment options and good health
practices, and promotes a timely, productive, and safe return to work.

Aynes & Brown, LLC will generate quarterly and annual
 reports which will document all activity for short term
disability management as well as outcomes and savings
   generated for each quarter. These reports include
               statistics on the following:

       Cases Reviewed per quarter/per year
                      Denials
                      Savings
      Days lost per case/per quarter/per year
                 Benefit Exclusions
  Breakdown by type of Short Term Disability Cases
             *diagnostic or procedural

Work Injury Management

Every year, millions of employees develop a disabling illness or
   injury that requires them to file a Work Injury claim. The
Department of Labor estimates that the direct and indirect cost
 to employers is at least $200 each day per absent employee.

  If you are facing employee health and productivity challenges,
our Disability Management Nurses can customize programs that
     specifically target work injury prevention and work injury
 management. We provide appropriate, timely, objective and cost
effective services that focus on the employee returning to work at
                     full health and productivity.

   We provide the same reporting features as our Short Term
Disability Management Program and can customize any report to
                      meet your needs.

Educational Offerings



We offer onsite educational programs specific to your needs. We
 have health and lifestyle coaches as well as registered nurses
         available to provide education and motivation.

Medical Review Services


     Medical Review Services is a program that includes
preutilization review, concurrent review, retrospective review,
 1st Level Appeals and 2nd Level Appeals. This program is
 designed to give feedback regarding medical necessity for a
                      variety of services.

     We will provide physician reviewers for the purpose of
determining the initial review for medical necessity. Reviewers
will utilize nationally accepted, corporate developed guidelines
 and physician expertise for the purpose of consistent quality
  determinations of medical necessity and appropriateness.

        Appeals will be provided through an outside
      non-Aynes & Brown, LLC affiliate physician with
    Board Certification in the clinical area of the request.

The Department of Labor estimates that the direct and indirect cost to employers is at least $200 each day per absent employee. If you are facing employee health and productivity challenges, our Disability Management Nurses can customize programs that specifically target work injury prevention and work injury management. We provide appropriate, timely, objective and cost effective services that focus on the employee returning to work at full health and productivity. We provide the same reporting features as our Short Term Disability Management Program and can customize any report to meet your needs. 13

We have health and lifestyle coaches as well as registered nurses available to provide education and motivation.   • 15. Medical Review Services Medical Review Services is a program that includes preutilization review, concurrent review, retrospective review, 1st Level Appeals and 2nd Level Appeals. This program is designed to give feedback regarding medical necessity for a variety of services. We will provide physician reviewers for the purpose of determining the initial review for medical necessity. Reviewers will utilize nationally accepted, corporate developed guidelines and physician expertise for the purpose of consistent quality determinations of medical necessity and appropriateness. Appeals will be provided through an outside non-Aynes & Brown, LLC affiliate physician with Board Certification in the clinical area of the request. 15
