SOCIAL SERVICES <ul><li>“ The Department of Social Services is comprised of Master’s level and Licensed Clinical Social Workers that are assigned to the hospital inpatient units and outpatient clinics. In support of CHCC’s mission and values, social workers provide a range of services that promote physical, emotional and psychological well being.” </li></ul><ul><li>CHCC social workers help guide and support patients and their families through the entire health care process. </li></ul><ul><li>Social workers assist families with personal, emotional, financial and family problems that may affect a child’s health. </li></ul><ul><li>Social workers help parents understand treatment plans and goals and offer crisis intervention, information and referral to community support agencies and other services.  </li></ul>
  3. 3. MISSION <ul><li>The mission of CHCC is “ to provide high quality, comprehensive healthcare services to children, regardless of their ability to pay, and to continuously improve the health and well-being of children.” </li></ul>

ORGANIZATIONAL STRUCTURE <ul><li>The hospital as a whole is overseen by the board of trustees. </li></ul><ul><li>The board of trustees then assign the President and the CEO; They are in charge of overseeing the day to day events of the hospital. </li></ul><ul><li>Every department within the hospital then has a Vice President and a Director, who supervise the staff that is under each department they’re assigned to. </li></ul>

COMMUNITY CONTEXT <ul><li>CHCC fit’s into the community in the following ways: </li></ul><ul><li>CHCC provides medical services to children ages 0-21 in the Central and San Joaquin Valley. </li></ul><ul><li>Most of our patients are on Medi-Cal or the California Children Services (CCS) program. </li></ul><ul><li>Social workers at CHCC are sensitive to cultural, social and religious beliefs. </li></ul><ul><li>Social workers partner with spiritual and chaplain services, interpreter services, child life, clinical psychology, rehabilitation and clinical nutrition to enhance the well-being of children. </li></ul>

POPULATIONS AT RISK <ul><li>The main population at risk that CHCC serves is children ages 0 – 21. </li></ul><ul><li>We also serve single parents, undocumented parents, and families with different social issues. </li></ul><ul><li>Every patient admitted to the hospital or who becomes a patient to one of the outpatient clinics is automatically assigned to a social worker. </li></ul>

FUNDING <ul><li>CHCC is a not-for-profit, state-of-the-art children’s hospital. </li></ul><ul><li>It is funded through: </li></ul><ul><li>Medi-Cal </li></ul><ul><li>Different foundations such as The National Hemophilia Foundation </li></ul><ul><li>Guilds </li></ul><ul><li>The Guilds have been raising funds for Children's Hospital for 61 years. </li></ul><ul><li>They are the largest financial contributors. </li></ul><ul><li>Radiothon </li></ul><ul><li>Donations </li></ul>
  10. 10. AGENCY’S SIGNIFICANCCE <ul><li>Children’s Hospital Central California has a vision of becoming the nation’s best children’s hospital. In an effort to achieve what this vision encompasses, we provide our services to ensure the well being and health of the child, regardless of their ability to pay. CHCC has one sole purpose, and that is to care for children. CHCC responds to children with pediatric emergencies, chronic diseases and acute injuries. </li></ul><ul><li>We serve families from Modesto to Bakersfield and from the Sierra Nevada Mountains to the Coast. This encompasses 40,000 square miles in the state of California. </li></ul>

AGENCY’S SIGNIFICANCE CONT… <ul><li>Our services are open to every family, regardless of their race, ethnicity, origin, financial status, etc… In order to provide our patients with the best possible services, we follow our values, which are Excellence, Compassionate Care, Integrity, Innovation, Collaboration, and Stewardship. Our values guide every decision and define our commitment and the actions supporting it. </li></ul><ul><li>Any child who has needed help or who will need help will have a neighbor to lend a hand.
