The issues advance health and wellness using innovative approaches and reach the most vulnerable populations in Imperial County. We are especially interested in those programs that reduce costs, improve quality and or increase capacity.
It is our mission support grants where we hope to be able to diversify our funding across the broader health and wellness areas. Unlike many foundations we aren’t looking to fund a particular program. Even though we ask about your innovative programs we want to work with organizations that show they can be innovative, or are ready to jump into the innovative/creative realm. We want to hear from you, through your application, what you think is the best practice for your area or how you are thinking you can begin to prepare for the future based on the changing trends and the fact that there will continue to be less money and more need in the future.
The first part of the application is the usual demographics: organization name, address, phone, fax, website (if you have one), mission statement & IRS letter of determination
For each of our priority areas you will answer to the best of your ability the % served. For example, of those you serve who are under 250% of poverty, what percentage is male and what percentage is female? Do the best you can even if you have to estimate the numbers. We recognize there will be some overlap in that your homeless population will be included in the 250% of poverty numbers as well. But not all people 250% of poverty and below are homeless.
