Care coordinators arrange for and authorize services, control payment for those services, improve availability and quality of services, contain costs by ensuring appropriate use of services, justify expenditure of funds and ensure that only appropriate enrolees utilize Health service. Care Coordinators also conduct evaluation of Patient Outcomes, Connection To Services, Adherence To Medication, Service Utilization and Cost of Service. Care coordinators should also monitor Patient Satisfaction and provide appropriate, comprehensive, and coordinated response to Patient's needs.
